Flight release form: Supplemental operations

This regulation specifies what information must be included on a flight release form for supplemental air carriers operating under Part 121.

The flight release can be in any format but must include essential flight details: company name, aircraft make/model/registration, flight number and date, crew names (including the designated pilot in command), departure/destination/alternate airports, route, minimum fuel in gallons or pounds, type of operation (IFR/VFR), and ETOPS diversion time if applicable.

The release must also contain or have attached the latest available weather reports and forecasts for destination and alternate airports at the time of signing. The pilot in command may include additional weather information as needed.

Importantly, if a supplemental operator is conducting domestic or flag operations, they must use the more detailed dispatch/flight release forms required for scheduled operations rather than this simplified version. This ensures appropriate operational control based on the type of flying being conducted.
